Saizen Questionnaire

While working at Tamooz Marketing Communications I created a simple Flash based Kiosk questionnaire, for Saizen the main goal of the application was to capture data from physicians attending a yearly convention.

The application would ask the user a series of question that relate to Saizen and then save the answers per user to external data base.
